Our direct primary care memberships use simple, transparent annual pricing so you can easily plan for your healthcare costs.

Adults age 30 and older:
$948 per year, plus tax

Children and young adults age 0–29:
$708 per year, plus tax

Dependent children age 0–26 added to a parent’s membership:
$348 per year, plus tax, per dependent

Available Discounts

Discounts of up to 10% may be available for:

Families or membership plans with three or more individuals
Memberships paid annually in advance
First responders, EMTs, police officers, firefighters, and nurses
Veterans and their families

These flat annual fees make it easier to budget for personalized, high-quality primary care without unexpected membership costs.

Referrals sent to specialists as needed (does not include specialist visit).

Medication management

Prescriptions sent to the pharmacy (does not include the cost of medications).

Lab draws

Blood draws performed in the clinic (fees from labs are separate).

Imaging orders

Imaging ordered as necessary (cost of imaging is separate).

Spirometry

Spirometry testing for lung function.

Suture and staple removal

Removal of staples or sutures from previous procedures.

Foley Removal

Removal of Foley catheters

Pap smear

Pap smears (pathology done by third-party, not included).

Phone Visit

Phone consultations for medical advice or follow-up.
